Registration for new Patients

Accepting new patients

We do not discriminate on the grounds of race, gender, social class

age,religion, sexual orientation or appearance, disability or medical condition

Any person who lives within our practice area and is ordinarily resident in the UK for more

than 6 months may be registered with the practice. 

To register please collect the appropriate forms from the reception staff

or

if you have confirmed you live within the Practice boundary

Please note it is now a requirement of Bromley Primary Care Trust that we ask patients

to supply:-

• Photo ID over 16 yrs (driving licence/passport)

• NHS number

• Proof of address (utility bill dated in last 3m)

• If new to the UK evidence of the 'right to remain'
